Overview:
We are seeking a dedicated and experienced Registered Nurse (RN) to join our Home Health team for a 13-week contract based in the Elizabethtown, PA area. This role is ideal for candidates with strong clinical and documentation skills who thrive in autonomous, patient-centered environments. The ideal candidate must live within 30 minutes of the assigned territory or secure nearby housing if traveling.

Key Responsibilities:

• Deliver skilled in-home nursing care in alignment with physician orders and patient needs
• Accurately complete OASIS-E documentation and submit same-day via Homecare Homebase (HCHB)
• Meet productivity expectations (5–7 points per day, approx. 20–25 cases per week)
• Travel within assigned territory (mileage reimbursed at $0.70/mile)
• Provide care including wound care, IV management, medication administration, patient teaching, and discharge planning
• Collaborate with interdisciplinary care teams to ensure continuity and quality of care
• Attend one mandatory in-office day weekly (typically Tuesday, Wednesday, or Thursday)

Schedule & Expectations:

• Full-time, 40 hours/week
• Day shift, flexibility required based on patient needs
• Weekend shifts may be required as indicated in the requisition
• No on-call responsibilities
• No overtime permitted
• All requested time off (RTO) must be approved in advance and made up

Requirements:

• Active RN license in Pennsylvania (or compact state)
• Minimum of 2 years of RN experience
• Home Health and OASIS-E experience required (experience must be post-January 2023)
• Proficiency with Homecare Homebase (HCHB)
• BLS certification (AHA)
• Reliable transportation and ability to document at the point of care (in patient’s home)
• Strong communication, time management, and clinical assessment skills

Additional Details:

• Care is provided in home-based settings, including private homes and ALFs
• Orientation consists of 2–3 days of training, including modules and team introduction
• Branch office hours: 8 AM – 5 PM (visits may occur before or after)
• Scrub color: Any professional attire
• PPE and parking details provided during onboarding
• Pay cycle: Monday–Sunday
