📋 Job Summary – RN Case Manager (Discharge Planning)

Facility : UMMC
Department : Case Management – General CM/Discharge Planning
Location : Jackson, MS
Start Requirement : Fingerprinting must be completed at least 10 days prior (remote) or the week prior (in person at Jackson/Grenada campus).
Schedule :

• Shifts : 5x8 hr shifts (non-negotiable), Monday–Friday, 8:00 AM–4:30 PM
• Weekends : None
• Holidays : Rotated as needed

✅ Job Requirements Must-Have Experience :

• RN Case Manager with 2+ years Acute Care Case Management experience
• Minimum 6 months at Level 1 Trauma Center or large academic medical center
• Discharge planning experience across varied inpatient units
• Strong understanding of discharge readiness
• EPIC charting experience (Required)
• NaviHealth experience a plus (currently used for referrals)
• EMR transition to EPIC Care Management planned over next year

Certifications :

• Required : RN license, BLS
• Preferred : CCM (Certified Case Manager)
• Education : BSN preferred , ADN considered

🏥 Patient & Department Details

• Caseload : 15–20 patients per day
• Focus : Discharge planning (no UR responsibilities)
• Coverage : All inpatient units
• Common Patients : Complex discharges, uninsured/underinsured, level 1 trauma patients
• Support : Collaborates with Social Work team (15 FT / 17 PT SWs)

Team Structure :

• Currently 15 Case Managers
• Goal: 21 Case Managers (18 general, 3 complex)
• No CM Manager currently – reports to Social Work Manager
• Oversight from Chief of Care Coordination

🩺 Other Info

• Flu Vaccine : Required (Only medical exemptions accepted with documentation of severe reaction)
• Scrubs/Dress Code : Any color scrubs OR business professional attire
• Orientation : 2 weeks (includes facility and department-specific training); CM will not work alone during orientation
• RT/Pharmacy Consultation : As needed
