Job Title: Home Health • LVN/LPN • Days

We are seeking a dedicated and experienced LVN/LPN for a 13•week contract role in home health care. This position requires a commitment of 32 hours per week, primarily during day shifts.

Responsibilities:

• Providing high•quality home health care to patients within a specific territory.

• Flexibility to float to another location if required.

• Potential for contract extension.

Requirements:

• Current MA LVN License.

• BLS Certification.

• Minimum of 2 years of experience in Home Health.

• Experience with OASIS; familiarity with Homecare Homebase (HCHB) is preferred but not required.

Additional Information:

• No on•call requirement.

• No specific holiday requirements.

• Scrub color is not specified.

• Personal Protective Equipment (PPE) details to be determined.

• Pay cycle runs from Sunday to Saturday.

• Patient ratio is typically 1:1.

• Orientation will be provided at our client's location.

Preferred Experience and Qualifications:

• Experience in home health care in the following areas: Allston, Brighton, Brookline, Dorchester, Jamaica Plain, Roxbury, South Boston.

• Experience in hospice care is a plus.
