SUMMARY

Under general supervision of a Director, is responsible for providing assistance to families in understanding illness and disabilities. Helps to resolve personal, financial, and environmental difficulties that add to the challenges of providing care for a child with special needs. Enables and empowers families to advocate appropriately. Works in accordance with Adapt Community Network policies, rules and requirements of applicable regulatory agencies and the standards of the profession.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The duties and responsibilities of the Family Resource Coordinator will include, but are not limited to the following:

• Assists Director in development of applicable standardized intake forms to track pertinent information i.e. client demographics, disability characteristics and needs.
• Participates in Intake process.
• Develops a plan for each family based upon information received during Intake process.
• Records progress on all active cases, monitors status and makes appropriate. recommendations as families' needs change.
• Develops collaborative relationships within community.
• Provides families with resources to be able to meet their needs.
• Provides interactive presentations for families and professionals.
• Promote awareness of library resources, services, and programs to the parents and staff of children in our program.
• Choose resources to enhance the library collection, as well as materials best suited for our population.
• Travels through region to facilitate viable learning activities and opportunities in within our school programs.
• Facilitate face to face meetings and workshops.
• Serves as liaison between Family Connect Program and special projects.
• Acts as a liaison with other programs to coordinate special projects services.
• Assures confidentiality of information about all of children and families receiving services.
• Moderates and facilitates sessions as needed.
• Creates flyers and communications for all webinars.
• Ability to work independently and handle multiple tasks simultaneously.
• Ability to manage time efficiently.
• Outreach for sessions internally and externally.
• The Parent Center specialist is the liaison with the directors and assistant directors for sending communications about family connect activities.
• Coordinates and assembles wish lists for all sites, places orders with an appropriate vendor, follows up with venders, tracks order and reconciles the general ledger.
• Assists Directors in ordering supplies for the 11 schools and the SHARE libraries.
• Conducts live and written translations during and before the webinar into Spanish.
• Requests checks for each vendor and follows up with the accounting and purchasing departments to ensure the vendors get paid.
• Keeps abreast of changes in field.
• Provides necessary program support/coverage
• Implements universal precautions at all times
• Maintains a clean, orderly, and safe work area
• Maintains inventory of supplies
• Maintains equipment and orders supplies as requested
• Performs other related duties as requested.

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. • Ability to read, write, speak, and understand English
• Good organizational skills and ability to meet deadlines
• Good interpersonal skills necessary to interact effectively with coworkers, employees, children and their families, and outside contacts.
• Proficiency in the use of a computer and knowledge of appropriate software
• Due to sensitive nature of the position, must be able to maintain the strictest confidentiality
• Ability to work independently and handle multiple tasks
• Ability to analyze problems and determine corrective measures

EDUCATION and/or EXPERIENCE

• Master's Degree in Social Work, Education, or other Human Services area from an accredited program.
• One year of related experience is preferred.

COMPENSATION: $65,000 - $70,000 Annually + Industry-Leading Benefits!
