Job Description

*Job Summary:*

The Medical Social Worker (MSW) is responsible for providing social work services to patients and families in the home setting. This role focuses on addressing the psychological, emotional, financial, and environmental needs that may impact the patient’s ability to achieve optimal health outcomes. The MSW collaborates with the interdisciplinary care team to assess needs, develop care plans, and provide counseling, support, and resource coordination.

*Key Responsibilities:*
* Conduct psychosocial assessments of patients and families to identify social, emotional, and environmental needs.
* Develop and implement individualized care plans in collaboration with the interdisciplinary team.
* Facilitate access to community resources such as financial assistance, housing, transportation, home-delivered meals, and support groups.
* Assist with advance directives, durable power of attorney, and other legal documents related to healthcare planning.
* Advocate for patients and families to ensure their needs are met within the healthcare system.
* Maintain timely and accurate documentation in compliance with federal, state, and agency guidelines.
* Participate in case conferences and coordinate care with nurses, therapists, and physicians.

*Qualifications:*
* Master’s Degree in Social Work (MSW) from an accredited program.
* Licensed as a Social Worker LMSW in Florida
* Knowledge of community resources and state/federal healthcare regulations.
* Excellent communication, assessment, and problem-solving skills.
* Ability to work independently and travel to patients' homes.
* Valid driver’s license and reliable transportation required.

Pay: $40.00 - $60.00 per hour

Expected hours: 8 – 24 per week
